- Name of traveler
- Name of location of intended stay in CNMI
- Date of arrival and departure (provide copy of ticket or itinerary)
- Purpose of visit
- Copy of passport
- Police clearance needed if coming from
- Indonesia
- Malaysia
- if require by the approving officials
- National Bureau of Investigation (NBI) required if coming from Philippines (must be valid within 6 months from issue date)
- Sponsor Information
- Affidavit of Support (provided by Immigration, must be notarized)
- Copy of passport or any other ID
- Copy of entry permit (if applicable)
- Copy of two (2) latest pay stubs (proof that sponsor earns at least $20,000 US Dollars or more a year) OR Bank statements (must indicate a maintenance of at least $3,000 US Dollars or more for THREE (3) consecutive months prior to arrival of vistor)
- If company is sponsoring, must provide company bank statement for Three (3) consecutive months. / BGRT
* If sponsor is unable to provide either #4 (on sponsor information), they may be required to post a cash bond upon the discretion of the Attorney General.
* Sponsor must also be residing in the CNMI.
* Request must be submitted three (3) weeks before intended date of arrival.
* There will be a fee of $100.00 per application when q request is submitted seven (7) days or less from intended date of arrival. This fee does not guarantee approval.
* Processing time of VEP is seven (7) to ten (10) working days from date of submission.
